Summary

Chest CT scans can identify lung disease in organ donors. Emphysema and interstitial lung disease found on CT scans negatively impact lung utilization, suggesting CT is valuable for donor assessment.

Background:

  • Chest computed tomography (CT) is increasingly used for lung donor assessment.
  • The effectiveness of CT in this context is not well-established.

Purpose of the Study:

  • To determine if chest CT imaging independently influences lung donor utilization decisions.
  • To investigate the association between CT-detected lung abnormalities and lung utilization rates.

Main Methods:

  • Retrospective cohort study of adult brain death donors (June 2011-November 2016).
  • Thoracic radiologist reviewed donor chest CT and X-ray images for structural lung disease (emphysema, ILD) and acute abnormalities (TLI).
  • Analysis of lung utilization based on initial partial pressure of oxygen (iPaO2) levels.

Main Results:

  • 445 (59.1%) donors had chest CTs; emphysema (13.7%), ILD (2.5%), TLI (7.2%) were found.
  • TLI positively associated with utilization (OR 2.23), while emphysema negatively associated (OR 0.18).
  • Multivariate analysis confirmed emphysema's negative association (OR 0.21); ILD findings precluded donation. CT-detected structural lung disease negatively impacted utilization (P=.0001).

Conclusions:

  • Chest CT findings of structural lung disease (emphysema, ILD) are significantly associated with lower lung utilization.
  • CT imaging may serve as a valuable tool complementing existing lung donor assessment criteria.
