Background:

Tonsillectomy is a common surgical procedure.
Postoperative hemiparesis is a rare but serious complication.

Observation:

A 32-year-old woman developed left-sided hemiparesis after elective tonsillectomy and general anesthesia.
Extensive initial investigations for neurological or metabolic causes were negative.

Findings:

The patient was diagnosed with hemiplegic migraine at a neurology follow-up.
This diagnosis explained the postoperative motor deficit.

Implications:

Hemiplegic migraine should be considered in the differential diagnosis of postoperative hemiparesis.
Early diagnosis and treatment of hemiplegic migraine are crucial for patient recovery.
