Summary

Recombinant factor VIIa (rFVIIa) is used for factor VII deficiency in cardiac surgery, but can cause clots. A patient with factor VII deficiency successfully underwent surgery without rFVIIa, showing current tests may not predict its need.

Background:

Observation:

  • A 53-year-old male patient with factor VII deficiency (activity 15.2%, INR 2.9) presented with critical coronary anatomy.
  • The patient underwent complex cardiac surgery (coronary artery bypass) and a subsequent thoracotomy with decortication.
  • Successful surgical management was achieved without the administration of rFVIIa.

Findings:

  • Standard laboratory tests, including prothrombin time/international normalized ratio (PT/INR) and factor VII activity assays, may not reliably indicate the necessity for rFVIIa therapy.
  • Thromboelastography (TEG) also did not predict the requirement for replacement therapy in this specific case.
  • This case highlights potential limitations of current diagnostic tools in guiding rFVIIa use.

Implications:

  • Current diagnostic methods may not adequately predict the need for rFVIIa in patients with factor VII deficiency undergoing major surgery.
  • Clinical decision-making for rFVIIa supplementation may require a more nuanced approach beyond standard laboratory values.
  • Further research is needed to identify more reliable predictors for rFVIIa therapy in surgical patients with factor VII deficiency to optimize treatment and minimize risks.
