Background:

A 58-year-old woman with no significant past medical history presented with chronic bilateral wrist and knee pain.
The pain was refractory to conservative treatments including physiotherapy and intra-articular steroid injections.

Observation:

Patient reported a remote tibia fracture from a motor vehicle accident.
Social history noted immigration from Malaysia.
Serologic work-up for inflammatory conditions yielded negative results.

Findings:

Radiographs of the hands and knees were obtained to evaluate the persistent joint pain.
Further diagnostic workup is indicated to determine the etiology of the refractory pain.

Implications:

This case highlights the importance of a comprehensive diagnostic approach for unexplained chronic joint pain.
Identifying the underlying cause is crucial for effective management and improving patient quality of life.
