In 1928, a German botanist Emil Heitz observed the moss nuclei with a DNA binding dye. He observed that while some chromatin regions decondense and spread out in the interphase nucleus, others do not. He termed them euchromatin and heterochromatin, respectively. This study documents the first case of CD5-positive primary cutaneous diffuse large B-cell lymphoma-leg type (PCDLBCL-LT). This rare lymphoma variant presented as a thigh nodule and was confirmed via immunohistochemistry.

Background:

  • Primary cutaneous B-cell lymphomas (PCBCL) are typically CD5 negative.
  • CD5 expression is rare in PCBCL, making CD5+ variants diagnostically challenging.

Observation:

  • A 71-year-old woman presented with a thigh nodule.
  • Histological examination revealed diffuse dermal infiltration by large lymphoid cells.

Findings:

  • Immunohistochemistry confirmed the large lymphoid cells were positive for CD5, CD20, CD79a, MUM1/IRF4, Bcl6, Bcl2, and cytoplasmic IgM/λ.
  • The cells were negative for CD3, CD56, CD23, CD21, CD10, CD30, cyclin D1, CD68, lysozyme, myeloperoxidase, and CD34.
  • The diagnosis of CD5-positive primary cutaneous diffuse large B-cell lymphoma-leg type (PCDLBCL-LT) was established.

Implications:

  • This case expands the known spectrum of PCDLBCL-LT.
  • Highlights the importance of CD5 immunostaining in the differential diagnosis of cutaneous B-cell lymphomas.
  • Further research is needed to understand the clinical behavior and treatment of CD5+ PCDLBCL-LT.
