Summary

Diffusion tensor imaging (DTI) showed minimal changes over time in amyotrophic lateral sclerosis (ALS) patients. White matter integrity may be significantly impacted before disease onset, limiting detectable DTI progression.

Background:

  • Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease.
  • Understanding white matter changes in ALS is crucial for disease monitoring.

Purpose of the Study:

  • To assess longitudinal changes in diffusion tensor imaging (DTI) parameters in ALS patients.
  • To correlate DTI findings with clinical progression measured by the ALS functional rating scale-revised (ALSFRS-R).

Main Methods:

  • 23 ALS patients underwent two MRI scans with DTI at 6-month intervals.
  • Voxel-based white matter tract-based spatial statistics (TBSS) and region-of-interest (ROI) analyses were performed.
  • Fractional anisotropy (FA) and mean diffusivity (MD) were analyzed and correlated with ALSFRS-R scores.

Main Results:

  • No significant changes in DTI parameters (FA, MD) were detected between the two scans using TBSS.
  • ROI analysis revealed small, non-significant changes in FA and MD over 6 months.
  • No significant correlation was found between DTI metrics and ALSFRS-R scores after correcting for multiple comparisons.

Conclusions:

  • This study found limited evidence of progressive white matter changes on DTI in ALS patients over a 6-month period.
  • The lack of significant DTI changes may indicate substantial white matter damage occurring before or early in disease presentation.
  • Further research is needed to understand the timeline of white matter alterations in ALS.
