In vitro fertilization (IVF) pregnancies are associated with a significantly higher risk of severe maternal morbidity (SMM). Multiple pregnancies play a substantial role in mediating this increased risk, particularly with autologous oocyte IVF.
Area of Science:
- Reproductive Medicine
- Maternal Health
- Epidemiology
Background:
- In vitro fertilization (IVF) is a widely used assisted reproductive technology.
- Severe maternal morbidity (SMM) represents a significant public health concern.
- The association between IVF and SMM, and the role of multiple pregnancies, requires further investigation.
Purpose of the Study:
- To investigate the association between IVF and SMM.
- To explore the mediating role of multiple pregnancy in the IVF-SMM relationship.
- To differentiate risks associated with autologous oocyte IVF (IVF-AO) and oocyte donation IVF (IVF-OD).
Main Methods:
- Population-based cohort-nested case-control study in six French regions (2012/13).
- Included 2540 women with SMM and 3651 controls.
- Used multivariable logistic regression and path analysis to assess associations and mediation by multiple pregnancy.
Main Results:
- IVF pregnancies showed a significantly higher risk of SMM (aOR=2.5).
- IVF-AO was associated with increased SMM risk (aOR=2.0), with 49.6% mediated by multiple pregnancy.
- IVF-OD showed a markedly higher SMM risk (aOR=18.6), with 21.6% mediated by multiple pregnancy.
Conclusions:
- The risk of SMM is elevated in IVF-conceived pregnancies.
- Multiple pregnancy is a significant mediator of SMM risk in IVF-AO.
- While IVF-OD showed higher risks, further research with wider confidence intervals is needed for confirmation.
