    Summary

    Thyroid hormone binding inhibitor (THBI) was found in most critically ill patients with nonthyroidal illnesses (NTI). This inhibitor significantly increased the dialyzable fractions of T4 and T3, indicating altered thyroid hormone transport.

    Background:

    • Nonthyroidal illnesses (NTI) can affect thyroid hormone levels and metabolism.
    • Thyroid hormone binding inhibitor (THBI) has been implicated in altered thyroid hormone transport.

    Purpose of the Study:

    • To investigate the presence and impact of THBI in patients with NTI using a competitive ligand binding assay (CLBA).

    Main Methods:

    • Employed CLBA to detect THBI in serum ether extracts from 25 NTI patients.
    • Measured serum total T4, total T3, albumin, and dialyzable fractions of T4 (DFT4) and T3 (DFT3).
    • Analyzed correlations between THBI, clinical parameters, and thyroid hormone fractions.

    Main Results:

    • THBI detected in 60% of NTI patients (single sample) and 88% (multiple samples).
