Summary

A rare anatomical variation involving the ulnar nerve passing through the transverse carpal ligament was identified. This transligamentous course can cause nerve compression symptoms and poses a risk during carpal tunnel surgery.

  • A transligamentous course of the ulnar nerve sensory branch through the distal transverse carpal ligament was discovered during carpal tunnel release.
  • Surgical release of this aberrant branch led to symptom improvement.

Implications:

  • This case highlights a previously unreported anatomical variation of the ulnar nerve.
  • Hand surgeons must be aware of this variant to prevent iatrogenic injury during carpal tunnel release procedures.
