Leaves can be only millimeters in width or tens of meters wide, depending on the environment. Background:

  • Giant multilocular prostatic cystadenoma is an exceptionally rare benign prostate tumor.

Observation:

  • A 50-year-old male presented with a 2-year history of abdominal distension and lower urinary symptoms.
  • Imaging revealed a large (30 cm) retroperitoneal mass with multiple septations, displacing pelvic organs.
  • Initial differential diagnoses included leiomyoma or perivascular epithelioid cell tumor, with biopsy showing a spindle cell tumor.

Findings:

  • Histopathological analysis confirmed the diagnosis of GMPC, revealing dilated prostatic glands within a fibromuscular stroma.
  • The resected tumor measured 33 × 23 × 10 cm and contained cystic cavities filled with dark fluid.
  • Immunohistochemical staining for prostate-specific antigen confirmed the prostatic origin of the epithelial lining.

Implications:

  • This case represents one of the largest reported instances of GMPC, emphasizing the need for consideration of this diagnosis in large pelvic retroperitoneal masses.
  • Preoperative diagnostic strategies should be carefully considered to differentiate GMPC from other retroperitoneal tumors.
  • Appropriate surgical planning is essential for the successful management of these rare and extensive tumors.
