Sentinel lymph node (SN) biopsy shows potential for intraoperative lymph node metastasis (LNM) assessment in early gastric cancer (EGC). However, complex lymphatic drainage and skip metastasis present significant challenges for clinical application.
Area of Science:
- Surgical Oncology
- Gastroenterology
- Pathology
Background:
- Early gastric cancer (EGC) is increasingly diagnosed, with a low incidence of lymph node metastasis.
- Minimally invasive surgical approaches are gaining interest for EGC treatment.
- Accurate intraoperative assessment of lymph node metastasis (LNM) is crucial but currently lacks precision.
Purpose of the Study:
- To evaluate the potential clinical application of sentinel lymph node (SN) biopsy in gastric cancer.
- To explore the feasibility of intraoperative LNM assessment using SN mapping.
- To clarify the role of SN biopsy in the context of minimally invasive surgery for EGC.
Main Methods:
- A comprehensive literature search was performed.
- The study reviewed existing research on SN biopsy techniques in gastric cancer.
- Analysis focused on the accuracy and challenges of SN mapping for LNM detection.
Main Results:
- Sentinel lymph node (SN) mapping has been explored for intraoperative detection of sentinel lymph nodes (SLNs) in EGC.
- Predicting LNM status through SN mapping is an area of active investigation.
- The literature search aimed to clarify the potential clinical utility of SN biopsy.
Conclusions:
- SN biopsy in gastric cancer is challenging due to complex lymphatic drainage and potential skip metastasis.
- Technical aspects, including tracer selection and metastasis verification, are demanding.
- SN navigation for gastric cancer remains controversial, necessitating further clinical trials.
