Summary

Offline computer-based digital education (OCDE) shows uncertain effects on doctors' knowledge and cognitive skills compared to traditional learning. However, OCDE may improve knowledge when compared to no intervention, with minimal impact on patient outcomes.

Purpose of the Study:

  • To systematically review the effectiveness of OCDE for improving medical doctors' knowledge, cognitive skills, and patient outcomes.
  • To assess the cost-effectiveness and adverse effects of OCDE.

Main Methods:

  • A systematic review following Cochrane methodology was conducted.
  • Searched major bibliographic databases from 1990 to August 2017.
  • Included 27 RCTs, 1 cRCT, and 1 quasi-RCT; meta-analysis was not feasible due to heterogeneity.

Main Results:

  • OCDE's effect on knowledge and cognitive skills compared to face-to-face learning is uncertain (very low to low grade of evidence).
  • OCDE may improve knowledge compared to no intervention, but its effect on cognitive skills is uncertain.
  • OCDE showed little to no effect on patient outcomes compared to face-to-face or no intervention.

Conclusions:

  • The impact of OCDE on medical doctors' knowledge and cognitive skills remains uncertain across different educational comparisons.
  • OCDE may enhance knowledge acquisition compared to no intervention, but evidence for cognitive skill improvement is inconclusive.
  • OCDE appears to have minimal to no impact on patient outcomes.
