Summary

Remote ischemic preconditioning (RIP) did not reduce complications or improve cardiac biomarkers in patients undergoing coronary artery bypass surgery. This cardioprotective method showed no benefit in this study population.

Background:

  • Coronary artery bypass surgery (CABG) with cardiopulmonary bypass (CPB) carries risks of myocardial injury.
  • Remote ischemic preconditioning (RIP) is a potential cardioprotective strategy.
  • Propofol is commonly used for anesthesia during cardiac procedures.

Purpose of the Study:

  • To evaluate the efficacy of RIP as a cardioprotective method during CABG with CPB and propofol anesthesia.
  • To assess the impact of RIP on intraoperative and postoperative complications.
  • To measure cardiac injury markers (Troponin I) and lactate levels.

Main Methods:

  • A randomized controlled trial involving 87 patients with ischemic heart disease undergoing CABG.
  • Patients were assigned to either RIP or a control group.
  • Complications, Troponin I, and lactate levels were assessed pre- and post-surgery.

Main Results:

  • No significant differences in intraoperative or early postoperative complications between the RIP and control groups.
  • Similar Troponin I and lactate levels were observed in both groups after surgery.
  • RIP did not demonstrate a protective effect in this patient cohort.

Conclusions:

  • Remote ischemic preconditioning does not appear to be effective in reducing complications or improving outcomes in patients undergoing CABG with CPB and propofol anesthesia.
