Summary

Freezing of swallowing (FOS) is a newly identified symptom in progressive supranuclear palsy-parkinsonism (PSP-P). This swallowing impairment may contribute to unexplained weight loss in patients with parkinsonism.

Purpose of the Study:

  • To report on the novel manifestation of freezing of swallowing (FOS) in patients with progressive supranuclear palsy-parkinsonism (PSP-P).

Main Methods:

  • Case series involving 3 patients diagnosed with probable PSP-P.
  • Clinical examination including assessment of freezing of gait.
  • Video fluoroscopy to evaluate swallowing function, focusing on oral and pharyngeal phases.

Main Results:

  • All 3 patients experienced significant recent weight loss.
  • Video fluoroscopy revealed nonfunctional, 6- to 8-Hz trembling movements during chewing and volitional swallowing, characteristic of freezing.
  • Impaired oral bolus transportation was observed, while the pharyngeal phase remained largely unaffected.

Conclusions:

  • Freezing of swallowing (FOS) is identified as a novel clinical feature of PSP-P.
  • FOS can have significant, potentially treatable, impacts on patient health and quality of life.
  • Consideration of FOS is recommended for patients with degenerative parkinsonism presenting with unexplained severe weight loss.
