Summary

Necrotizing enterocolitis (NEC) in full-term infants presents differently and has distinct outcomes compared to preterm infants. This suggests NEC may be a separate clinical condition in term neonates.

Background:

  • Necrotizing enterocolitis (NEC) is a known complication of prematurity.
  • Limited research exists on NEC in full-term infants.
  • This study addresses the clinical presentation and outcomes of NEC in term neonates.

Purpose of the Study:

  • To characterize the clinical presentation of NEC in full-term (FT) infants.
  • To compare the outcomes of FT infants with NEC to those of preterm (PT) neonates.
  • To investigate if NEC in FT infants represents a distinct clinical entity.

Main Methods:

  • Retrospective chart review of infants diagnosed with NEC (Modified Bell stage ≥2) over 10 years.
  • Comparison of outcomes and comorbidities between FT (gestational age >37 weeks) and PT (gestational age <37 weeks) infants.
  • Level of Evidence: III.

Main Results:

  • Out of 170 infants, 28 (17%) were FT.
  • FT infants were more likely to have had prior cardiac surgery (64% vs. 8%).
  • PT infants had higher rates of surgical intervention (59% vs. 18%), Bell stage 3 disease (82% vs. 43%), vasopressor support (42% vs. 21%), and ventilatory support (75% vs. 43%).

Conclusions:

  • Full-term neonates with NEC exhibit different disease patterns and outcomes.
  • NEC in FT infants may be a distinct clinical entity compared to NEC in PT infants.
  • Further research is warranted to understand these differences.
