Summary

Selective internal radiotherapy (SIRT) with yttrium-90 causes DNA damage in immune cells of liver cancer patients. This DNA damage correlates with reduced lymphocyte proliferation, indicating impaired immune function after SIRT treatment.

Background:

  • Selective internal radiotherapy (SIRT) using yttrium-90 is effective for non-resectable hepatic malignancies.
  • Previous studies suggest SIRT may impair immune function.

Purpose of the Study:

  • To quantify DNA lesions in peripheral blood mononuclear cells (PBMCs) of SIRT patients.
  • To correlate DNA double-strand breaks (DSBs) with cellular immune responses post-SIRT.

Main Methods:

  • Assessed γH2AX and 53BP1 foci (markers of DSBs) in PBMCs from ten SIRT patients.
  • Measured lymphocyte proliferation via phytohemagglutinin stimulation.
  • Analyzed samples before, 1 hour, and 1 week after SIRT.

Main Results:

  • γH2AX and 53BP1 foci significantly increased post-SIRT compared to baseline (p < 0.01).
  • Baseline foci levels were higher than in healthy controls; lymphocyte proliferation was already reduced.
  • Post-SIRT, lymphocyte proliferation further decreased and inversely correlated with DNA foci levels (r < -0.65, p < 0.05).

Conclusions:

  • SIRT induces significant DNA damage in PBMCs of liver cancer patients.
  • This DNA damage is associated with impaired in vitro immune function, potentially due to unrepaired DSBs or cell cycle arrest.
