In 1928, a German botanist Emil Heitz observed the moss nuclei with a DNA binding dye.     Background:

    • Volar plate injuries of the proximal interphalangeal joint (PIPJ) are common, often resulting from sports or workplace incidents.
    • Current conservative treatment involves dorsal block splinting (DBS) in 30° flexion to promote ligament healing.
    • DBS in 30° flexion can lead to complications like joint flexion deformities and hyperextensibility.

    Purpose of the Study:

    • To evaluate the efficacy of dorsal block splinting (DBS) in neutral position versus 30° flexion for treating stable volar plate injuries of the PIPJ.
    • To determine if neutral position splinting reduces complications and improves functional outcomes compared to 30° flexion splinting.

    Main Methods:

    • A retrospective review of 125 patients with stable volar plate injuries of the PIPJ was conducted.
    • Patients were treated with either DBS in neutral position (20 patients) or 30° flexion (105 patients).
    • Data collected included hand therapy sessions, time to discharge, PIPJ active range of motion, and pain levels.

    Main Results:

    • No significant demographic differences were observed between the two groups.
    • Patients treated with DBS in neutral position required fewer hand therapy sessions and less therapy time before discharge.

    Conclusions:

    • Dorsal block splinting (DBS) in neutral position for PIPJ volar plate injuries effectively reduces flexion deformities without increasing hyperextensibility.
    • Neutral position splinting leads to decreased hand therapy demands and may facilitate a quicker return to daily activities.
    • This conservative approach offers potential for improved patient function and fewer complications following volar plate injuries.
