In any solution, the value of pKa indicates whether an acid is completely dissociated or not. A negative pKa corresponds to a stronger acid, whereas a positive pKa corresponds to a weaker acid. Consider the reaction between ammonia and an ethoxide ion. In this reaction, ethanol with a pKa of 15.9 is a stronger acid than ammonia with a pKa of 38. Recall that the strong acid forms a weak conjugate base, and a weak acid forms a strong conjugate base. Hence, the ethoxide ion is a weak base.

The benzylic position describes the position of a carbon atom attached directly to a benzene ring. Benzene by itself does not undergo oxidation. In contrast, the benzylic carbon is quite reactive in the presence of strong oxidizing agents such as KMnO4 or H2CrO4. Therefore, alkylbenzenes are readily oxidized to benzoic acid, irrespective of the type of alkyl groups.

Summary

Optical coherence tomography (OCT) offers objective markers for allergic contact dermatitis (ACD) patch testing. OCT measurements of attenuation coefficient and blood flow correlate with reaction severity, improving diagnostic accuracy.

Purpose of the Study:

  • Evaluate micromorphological changes in patch test reactions using OCT.
  • Identify objective, quantitative OCT markers correlating with clinical grading.

Main Methods:

  • 25 patch test reactions (grades 0, 1+, 2+) from 7 patients were analyzed with OCT.
  • Qualitative and quantitative OCT parameters were assessed.

Main Results:

  • Increased epidermal thickness and density observed in reactions.
  • Well-demarcated, signal-free cavities noted in severe reactions.
  • Attenuation coefficient and cutaneous blood flow significantly increased with reaction severity (P < 0.01).

Conclusions:

  • OCT-derived attenuation coefficient and blood flow are promising objective markers for patch test reactions.
  • OCT can enhance diagnostic accuracy in interpreting allergic reactions.
  • OCT may improve differentiation of clinical scoring in patch testing.
