Summary

Prognosis for pelvic Ewing sarcoma (ES) is poor, but local control is achievable with surgery or radiotherapy. Necrosis response is a key factor for survival in these challenging ES cases.

Background:

  • Pelvic Ewing sarcoma (ES) has a worse prognosis than limb ES despite multidisciplinary therapies.
  • Local control and survival remain significant challenges in managing pelvic ES.

Purpose of the Study:

  • To evaluate prognostic factors for overall survival (OS) and disease-free survival (DFS) in non-metastatic pelvic ES.
  • To assess the impact of local treatment, margins, necrosis, and sacrum involvement on outcomes.

Main Methods:

  • Retrospective analysis of 21 non-metastatic pelvic ES patients.
  • Evaluation of local treatment types (surgery, radiotherapy), margins, necrosis, and sacrum involvement.
  • Statistical analysis of OS and DFS.

Main Results:

  • Average follow-up was 46.3 months.
  • Necrosis response was the only significant prognostic factor for OS and DFS (p=0.0132, p=0.0086).
  • 5-year OS was 40.1%; recurrence was low (1/21).

Conclusions:

  • Local control is comparable with surgery, surgery plus radiotherapy, or definitive radiotherapy for pelvic ES.
  • Surgery plus radiotherapy may benefit cases with large tumors, poor necrosis response (<90%), or sacrum involvement.
  • Poor neoadjuvant therapy response is a significant risk factor for local control and OS.
