Summary

The Pipeline Embolization Device (PED) shows promising safety and effectiveness for treating distal cerebral circulation aneurysms. This study found good clinical outcomes with no increased complications in these challenging cases.

Background:

  • The Pipeline Embolization Device (PED) is increasingly used beyond its initial indications for complex intracranial aneurysms.
  • Its application has expanded to include smaller, fusiform, and dissecting aneurysms, as well as blister-like aneurysms.

Purpose of the Study:

  • To evaluate the safety and clinical outcomes of using the PED in off-label distal cerebral circulation (DCC) arteries.
  • To assess aneurysm occlusion rates and complications associated with PED treatment in DCC.

Main Methods:

  • A retrospective analysis of 437 patients treated with PED between 2011 and 2016.
  • Twenty-three patients with DCC aneurysms were identified and analyzed for aneurysm characteristics, procedural details, and clinical outcomes.
  • Multivariable logistic regression and propensity score conditioning were used to control for confounding factors.

Main Results:

  • Out of 437 patients, 23 (5.2%) received PED for DCC aneurysms, including those in the middle cerebral, posterior cerebral, anterior cerebral, and posterior inferior cerebellar arteries.
  • No significant associations were found between PED use in DCC and aneurysm occlusion or thromboembolic complications.
  • Ninety-five percent (22/23) of patients treated with PED in DCC arteries achieved a good clinical outcome.

Conclusions:

  • Treatment of distal cerebral circulation aneurysms with the PED is technically demanding due to vessel anatomy.
  • The study supports the safety and efficacy of flow diverters, like the PED, for treating various distal intracranial aneurysms.
  • PED utilization in DCC arteries demonstrates favorable clinical outcomes and supports its expanded use in neurovascular interventions.
