    Background:

    • Intravesical chemotherapy is a standard treatment for superficial bladder tumors.
    • The optimal timing for chemotherapy instillation after transurethral resection (TUR) remains undetermined.

    Observation:

    • A patient with superficial bladder cancer received immediate intravesical chemotherapy with mitomycin C post-TUR.
    • The chemotherapy dose was relatively high.

    Findings:

    • The patient developed bone marrow failure within two weeks of treatment.
    • A potential causal relationship between immediate high-dose mitomycin C and bone marrow failure is suggested.

    Implications:

    • This case highlights a potential risk associated with immediate post-TUR intravesical chemotherapy.
    • Further investigation is warranted to determine the safe and effective timing and dosage of intravesical chemotherapy after TUR for bladder cancer.
