Summary

Increased T2 signal in the proximal patellar tendon on knee MRI is common, but rarely indicates patellar tendinopathy. This finding on MRI may not be pathological without clinical symptoms.

Purpose of the Study:

  • To assess the clinical significance of T2 signal hyperintensity in the proximal patellar tendon on knee MRI.
  • To determine if this MRI finding correlates with clinical patellar tendinopathy.

Main Methods:

  • Retrospective review of 100 knee MRIs performed on 3-Tesla scanners.
  • Assessment of proximal patellar tendon T2 signal intensity (none, mild, moderate, severe).
  • Correlation with clinical notes for patellar tendinopathy symptoms.

Main Results:

  • 66% of knee MRIs showed increased T2 signal in the proximal patellar tendon.
  • Only 4.5% of patients with this MRI finding had clinical signs of patellar tendinopathy.

Conclusions:

  • Increased T2 signal in the proximal patellar tendon is a frequent MRI finding.
  • This finding is rarely associated with clinical patellar tendinopathy.
  • T2 signal hyperintensity alone may not signify pathology without corroborating clinical symptoms.
