Background:

  • Cryopreservation of human oocytes is crucial for fertility preservation in women undergoing gonadotoxic therapies or facing ovarian senescence.
  • Current methods for predicting oocyte cryopreservation resistance rely on subjective morphological assessments.

Purpose of the Study:

  • To investigate the relationship between oocyte morphometric characteristics before vitrification and after warming with successful fertilization via intracytoplasmic sperm injection (ICSI).

Main Methods:

  • A prospective cohort study involving 28 infertile women and 71 oocytes.
  • Morphometric assessments included oocyte diameter, perivitelline space (PS), zona pellucida (ZP), and first polar body (PB) before and after cryopreservation.
  • Oocytes were vitrified and warmed, followed by ICSI for fertilization assessment.

Main Results:

  • Oocyte survival rate after warming was 55% (27 out of 49).
  • Pre-vitrification morphometric measures did not differ significantly between surviving and non-surviving oocytes.
  • Oocytes failing fertilization showed significant post-warming total diameter enlargement and PS shrinking compared to successfully fertilized oocytes.

Conclusions:

  • Fresh oocyte morphometric characteristics do not predict survival after cryopreservation.
  • Post-warming oocyte enlargement and perivitelline space reduction are associated with subsequent fertilization failure.
