Summary

Cervical elastography strain patterns, including the elastography index and strain pattern score, are superior to cervical length alone in predicting spontaneous preterm delivery (SPTD). Combining these markers offers enhanced predictive accuracy for SPTD.

Background:

  • Spontaneous preterm delivery (SPTD) remains a significant challenge in perinatal care.
  • Accurate prediction of SPTD is crucial for timely intervention and improved neonatal outcomes.
  • Current predictive markers, such as cervical length (CL), have limitations in sensitivity and specificity.

Purpose of the Study:

  • To evaluate the efficacy of cervical elastography strain patterns as a predictive marker for spontaneous preterm delivery (SPTD).
  • To compare the predictive performance of elastography-derived parameters against traditional cervical length measurements.

Main Methods:

  • A case-control study involving 335 pregnant women (20-34 weeks gestation) using transvaginal ultrasound.
  • Acquisition of cervical length (CL), strain ratio, elastography index, and strain pattern (SP) score.
  • Comparison of data from 50 preterm deliveries with 285 normal pregnancies.

Main Results:

  • Elastography index and SP score were significantly different between preterm and normal pregnancies (p < 0.001).
  • The elastography index (AUC 0.8059) and SP score (AUC 0.7716) demonstrated higher predictive potential than CL alone (AUC 0.7631).
  • A combination of all assessed parameters yielded the highest predictive accuracy (AUC 0.8987).

Conclusions:

  • Higher elastography index and SP scores are strongly correlated with an elevated risk of SPTD.
  • Cervical elastography parameters are superior to CL measurement for predicting SPTD.
  • A combined "Cervical Index" utilizing elastography data offers a promising tool for SPTD prediction.
