Primary peritoneal carcinosarcoma: A report of two cases

Background:

• Carcinosarcomas, also known as malignant mixed Mullerian tumors (MMMTs), are rare neoplasms characterized by both carcinomatous and sarcomatous components.
• While most MMMTs originate in the female genital tract (ovaries, uterus, fallopian tubes), extragenital occurrences are exceptionally rare, with fewer than 40 cases documented.
• Primary peritoneal carcinosarcomas represent an extremely uncommon subset of these tumors.

Observation:

• This report details two cases of primary peritoneal carcinosarcoma.
• Case 1: An 81-year-old woman with pelvic peritoneal carcinosarcoma (heterologous type) underwent incomplete surgery without adjuvant chemotherapy and experienced rapid disease progression.
• Case 2: A 76-year-old woman with abdominal peritoneal carcinosarcoma (homologous type) underwent optimal debulking surgery followed by six cycles of paclitaxel and carboplatin chemotherapy, remaining disease-free for six months post-treatment.

Findings:

• Treatment outcomes varied significantly between the two presented cases.
• The patient receiving aggressive multimodal therapy (cytoreductive surgery and platinum-based chemotherapy) demonstrated a favorable short-term response.
• In contrast, the patient with less aggressive management experienced rapid disease progression.

Implications:

• Complete cytoreductive surgery combined with paclitaxel and carboplatin chemotherapy may represent a viable therapeutic strategy for managing primary peritoneal carcinosarcomas.
• These findings underscore the importance of aggressive treatment protocols for rare extragenital carcinosarcomas.
• Further research is warranted to establish optimal treatment guidelines for this rare malignancy.
