Summary

Background:

  • Severe lower-limb valgus deformity (>20°) presents unique challenges in total knee replacement (TKR).
  • Limited research exists on the impact of valgus severity on TKR outcomes.

Purpose of the Study:

  • To compare operative data, complications, implant survival, and clinical/radiological results between patients with >20° valgus and those with 10-20° valgus undergoing TKR.
  • To assess the necessity of greater implant constraint in severe valgus deformities.
  • To evaluate the safety and efficacy of TKR in cases of significant valgus.

Main Methods:

  • A multicenter retrospective case-control study included 53 patients with >20° valgus and 53 matched controls with 10-20° valgus.
  • Data collected included operative approach, TKR type, complications, implant survival, Oxford score, and HKA angle.
  • Statistical analysis compared outcomes between the two groups, with a mean follow-up of 8 years.

Main Results:

  • Greater implant constraint was more frequent in the >20° valgus group (p=0.03).
  • No significant differences were observed in complication rates (13.2% vs. NS) or 8-year implant survivorship (95.12% vs. 94.9%) between groups.
  • Clinical outcomes (Oxford score) and radiological alignment (HKA angle) were comparable between the severe and moderate valgus groups.

Conclusions:

  • The hypothesis that severe valgus deformity requires greater constraint was partially confirmed.
  • TKR in severe valgus deformity (>20°) can achieve low complication rates and comparable implant survival to less severe cases.
  • These findings support the use of TKR for significant lower-limb valgus deformities with careful consideration of implant selection.
