Purpose of the Study:

  • To identify the preferred provider education method for a pharmacist-led type 2 diabetes CDTM protocol.
  • To evaluate pharmacist and provider satisfaction with the implemented CDTM protocol.

  • A prospective cohort study was conducted in a health system involving pharmacists and providers in primary care clinics.
  • Providers received education via email, a provider meeting, or a personalized provider report card.
  • Patient referrals to pharmacists and provider/pharmacist satisfaction were assessed over a 2-month period.

  • 54 patients were referred for pharmacist management under the CDTM protocol.
  • The personalized provider report card method led to the majority of patient referrals (89%).
