Background:

  • Deep-brain stimulation (DBS) is a therapeutic option for essential tremor.
  • Directional DBS may offer advantages for targeting specific brain structures like the ventral intermediate (VIM) nucleus of the thalamus.
  • Medial current spread and stimulation of vestibular-cerebellar-thalamic pathways can exacerbate ataxia in DBS for essential tremor.

Observation:

  • A patient with essential tremor and refractory head tremor underwent bilateral VIM DBS using segmented leads.
  • Directional stimulation programming was compared with non-directional programming in this patient.
  • Video analysis assessed changes in ataxia during different stimulation paradigms.

Findings:

  • Directional DBS programming did not exacerbate the patient's baseline ataxia.
  • Non-directional DBS programming consistently worsened ataxia.
  • Segmented leads provided enhanced programming flexibility for VIM DBS.

Implications:

  • Directional DBS offers a potential strategy to mitigate ataxia, a common complication of VIM DBS for essential tremor.
  • Optimized lead design and programming can improve the safety and efficacy of DBS therapy.
  • This approach may enhance quality of life for patients with essential tremor undergoing DBS.
