Summary

For MRI-negative operculoinsular epilepsy, combining morphometric analysis program (MAP), magnetoencephalography (MEG), and subtraction ictal SPECT coregistered to MRI (SISCOM) offers the highest diagnostic yield. MAP provided unique information when other noninvasive tests were inconclusive.

  • Twenty-two patients with MRI-negative operculoinsular epilepsy and focal ictal onset on intracranial EEG were included.

  • Seizure outcomes were assessed using the Engel classification at 1 and 2 years post-resection.

Conclusions:

  • Noninvasive modalities, particularly their combinations, are valuable for evaluating nonlesional operculoinsular epilepsy.
  • The combination of MAP, MEG, and SISCOM offers comprehensive structural, interictal, and ictal localization information.
  • MAP is particularly useful for identifying unique information in challenging cases where other tests are negative.
