Background:

Purpose of the Study:

  • To compare outcomes for patients undergoing appendicectomy before and after the implementation of an ASU model.
  • To evaluate the impact of the ASU model on surgical care efficiency and patient safety.

Main Methods:

  • Retrospective cohort study comparing patients from 2.5 years before (Traditional group) and after (ASU group) the ASU model implementation.
  • Primary outcomes included length of stay, time to theatre, and in-hours operations.
  • Secondary outcomes assessed complications, open appendicectomies, consultant involvement, ICU admission, and 30-day re-presentation rates.

Main Results:

  • The ASU group (749 patients) showed a greater proportion of in-hours operations (79% vs. 72%) compared to the Traditional group (465 patients).
  • The ASU model was associated with significantly fewer complications (6% vs. 9%) and reduced rates of open or conversion-to-open appendicectomies (1% vs. 4%).
  • Superior rates of consultant involvement in theatre were observed in the ASU group (56% vs. 21%), with no significant changes in length of stay, time to theatre, ICU admission, or re-presentation rates.

Conclusions:

  • Implementation of the ASU model at this institution led to more in-hours operations for appendicectomy.
  • The ASU model was associated with improved patient safety, evidenced by fewer complications and reduced open appendicectomies.
