Summary

A rare holocord syrinx in a child, caused by Chiari I malformation, was successfully treated with craniocervical decompression. This surgery resolved the syrinx and reversed the foot drop.

Background:

  • Holocord syrinx is a rare spinal cord anomaly characterized by a fluid-filled cavity extending throughout the entire length of the spinal cord.
  • Chiari I malformation, a structural defect where cerebellar tonsils protrude into the spinal canal, is a known cause of syringomyelia and syrinx formation.

Observation:

  • A 4-year-old boy presented with a 2-week history of progressive right-sided foot drop, indicating a neurological deficit.
  • Magnetic Resonance Imaging (MRI) revealed a holocord syrinx extending throughout the spinal cord.
  • The syrinx was found to be secondary to an underlying Chiari I malformation.

Findings:

  • The patient underwent successful craniocervical decompression surgery to address the Chiari I malformation.
  • Post-operatively, the holocord syrinx significantly decreased in size.
  • The neurological deficit, specifically the right-sided foot drop, completely resolved after surgical intervention.

Implications:

  • This case highlights an unusual presentation of holocord syrinx in a pediatric patient.
  • Craniocervical decompression is an effective treatment for pediatric holocord syrinx secondary to Chiari I malformation.
  • Early diagnosis and surgical intervention can lead to complete neurological recovery in affected children.
