Hypoglycaemic clamp with 2-deoxy-2-[18F]fluorodeoxyglucose (18F-FDG) PET/CT enhances adipose tissue imaging. Type 2 diabetes impairs visceral adipose tissue glucose uptake, highlighting its role in disease pathogenesis.
Area of Science:
- Endocrinology
- Nuclear Medicine
- Metabolic Research
Background:
- Adipose tissue glucose uptake is crucial for type 2 diabetes pathogenesis.
- Assessing adipose tissue glucose metabolism via 2-deoxy-2-[18F]fluorodeoxyglucose (18F-FDG) PET/CT is limited by low tracer uptake kinetics.
- Hypoglycaemic clamp conditions were investigated to enhance 18F-FDG uptake in adipose tissue.
Purpose of the Study:
- To test if [18F]FDG PET/CT during a hypoglycaemic clamp improves adipose tissue tracer uptake.
- To compare adipose tissue glucose handling between individuals with and without type 2 diabetes.
- To establish [18F]FDG PET/CT during hypoglycaemic clamp as a research tool for adipose tissue metabolism.
Main Methods:
- Participants with or without type 2 diabetes, who were overweight, underwent either a hyperinsulinaemic hypoglycaemic clamp or a hyperinsulinaemic euglycaemic clamp (n=5 per group).
- Tracer uptake was quantified using [18F]FDG PET/CT.
- Statistical analysis compared glucose uptake in visceral adipose tissue, skeletal muscle, liver, and pancreas between groups.
Main Results:
- Hypoglycaemic clamping significantly increased [18F]FDG uptake in the visceral adipose tissue of healthy participants (P=0.002).
- During hypoglycaemia, visceral adipose tissue glucose uptake was significantly lower in type 2 diabetic participants compared to healthy participants (P<0.0005).
- No significant differences in [18F]FDG uptake were observed in skeletal muscle, liver, or pancreas between the groups.
Conclusions:
- [18F]FDG PET/CT during a hypoglycaemic clamp is a promising tool for evaluating adipose tissue glucose metabolism.
- A specific impairment in visceral adipose tissue [18F]FDG uptake was observed in type 2 diabetes.
- This suggests an underappreciated role for adipose tissue glucose handling in the pathogenesis of type 2 diabetes.
