Summary

Cluster headache attacks significantly increase suicidality risk. Longer disease duration and higher scores on the Headache Impact Test (HIT-6) and Patient Health Questionnaire (PHQ-9) are associated with increased suicidal ideation during attacks.

Background:

  • Cluster headache is a debilitating neurological disorder.
  • Suicidality is a serious concern in patients with chronic pain conditions.

Purpose of the Study:

  • To investigate the prevalence of suicidality in cluster headache patients.
  • To identify factors associated with increased suicidality during cluster headache attacks.

Main Methods:

  • A multicenter study involving 193 cluster headache patients.
  • Assessment of passive and active suicidal ideation, suicide plans, and suicide attempts during and between attacks.
  • Logistic regression analysis to identify factors linked to high ictal suicidality.

Main Results:

  • During attacks, 64.2% reported passive suicidal ideation, 35.8% active suicidal ideation, 5.8% suicidal planning, and 2.3% suicide attempts.
  • Factors associated with high ictal suicidality included longer disease duration, higher Headache Impact Test (HIT-6) scores, and higher Patient Health Questionnaire (PHQ-9) scores.
  • Suicidality was significantly higher during attacks compared to interictal or between-bouts periods.

Conclusions:

  • Cluster headache attacks are associated with a high risk of suicidality.
  • Intensive treatment to reduce the burden of cluster headache may help mitigate suicide risk.
