Insights

Aromatase inhibitors (AIs) did not improve adult height in boys with idiopathic short stature (ISS). Long-term follow-up confirmed similar adult heights and vertebral deformity rates between AI-treated and placebo groups, indicating no significant benefit or harm.

Area of Science:

  • Pediatric Endocrinology
  • Growth Disorders
  • Pharmacological Interventions

Background:

  • Aromatase inhibitors (AIs) are used off-label for idiopathic short stature (ISS) in boys.
  • Limited data exist on the long-term effects of AIs on adult height in this population.

Purpose of the Study:

  • To evaluate adult height outcomes in boys with ISS treated with letrozole (Lz) or placebo (Pl).
  • To assess long-term skeletal safety and vertebral deformity changes following AI treatment.

Main Methods:

  • Randomized, double-blind, placebo-controlled trial involving boys treated with Lz or Pl for 2 years.
  • Adult height and spine MRI data were collected for long-term follow-up.
  • Bone age was assessed at treatment cessation.

Main Results:

  • Adult heights were comparable between the Lz and Pl groups (164.8 cm vs. 163.7 cm).
  • Adult heights in both groups did not significantly differ from predicted adult heights at baseline.
  • Vertebral deformity frequencies were similar between groups (29% Lz vs. 22% Pl), with no clear enrichment of comorbidities.

Conclusions:

  • Letrozole treatment did not significantly improve adult height in pre- or early-pubertal boys with ISS.
  • Previously observed vertebral deformities showed amelioration during follow-up.
  • Letrozole therapy appears to be skeletally safe in children and adolescents with ISS.

pre-mRNA Processing02:01

pre-mRNA Processing

In eukaryotic cells, transcripts made by RNA polymerase are modified and processed before exiting the nucleus. Unprocessed RNA is called precursor mRNA or pre-mRNA to distinguish it from mature mRNA.
Once about 20-40 ribonucleotides have been joined together by RNA polymerase, a group of enzymes adds a “cap” to the 5’ end of the growing transcript. In this process, a 5’ phosphate is replaced by modified guanosine that has a methyl group attached to it (7-Methyl...

Increased Body Temperature01:25

Increased Body Temperature

A body temperature above  38°C  (100.4 °F) is known as fever or pyrexia, and a person with fever is termed 'febrile.' Typically, the hypothalamus, a part of the brain that acts as the body's thermostat, regulates body temperature through a thermoregulatory setpoint. It receives signals from cold and warm thermal receptors throughout the body and adjusts the body's temperature accordingly. Fever occurs when this hypothalamic setpoint is altered, usually in...

Increased pulse rate01:17

Increased pulse rate

Tachycardia is a condition marked by an abnormally fast or irregular heart rate, surpassing the typical resting rate. In adults, tachycardia is characterized by a pulse rate ranging from 100 to 180 beats per minute. The increased heart rate can result in inadequate blood flow to various body parts, ultimately diminishing the oxygen supply to organs and tissues.
Many factors can elevate the risk of developing tachycardia. These include advanced age, a family history of arrhythmias, and an...

Chromatin Structure Regulates pre-mRNA Processing02:41

Chromatin Structure Regulates pre-mRNA Processing

In eukaryotic cells, nascent mRNA transcripts need to undergo many post-transcriptional modifications to reach the cell cytoplasm and translate into functional proteins. For a long time, transcription and pre-mRNA processing were considered two independent events that occur sequentially in the cell. However, it has now been well established that transcription and pre-mRNA processing are two simultaneous processes that are precisely regulated inside the cell.
The chromatin structure, especially...

Adult Stem Cells01:33

Adult Stem Cells

Stem cells are undifferentiated cells that divide and produce more stem cells or progenitor cells that differentiate into mature, specialized cell types. All the cells in the body are generated from stem cells in the early embryo, but small populations of stem cells are also present in many adult tissues including the bone marrow, brain, skin, and gut. These adult stem cells typically produce the various cell types found in that tissue—to replace cells that are damaged or to continuously...
