Summary

Catheter ablation effectively reduces symptoms for left atrial flutter, a condition often following cardiac procedures. However, nearly half of patients experience recurrent arrhythmias during midterm follow-up.

Background:

  • Left atrial flutter (LAF) affects up to 10% of patients post-pulmonary vein isolation or cardiac surgery.
  • LAF is typically symptomatic, poorly responsive to antiarrhythmic drugs, and often requires catheter ablation.

Purpose of the Study:

  • To investigate midterm freedom from recurrent arrhythmia after catheter ablation for left atrial flutter (LAF).

Main Methods:

  • Retrospective analysis of 53 patients undergoing catheter ablation for LAF from January 2014 to April 2017.
  • Data collected from the National Danish Ablation Registry.
  • Midterm follow-up (mean 20 ± 12 months) assessed arrhythmia recurrence and symptom scores (EHRA).

Main Results:

  • 89% of patients had prior cardiac procedures (ablation or surgery).
  • Acute procedural success (non-inducibility) was achieved in 85% of patients.
  • During midterm follow-up, 26 patients (49%) experienced recurrent atrial arrhythmia, though symptom scores significantly improved (p < .001).

Conclusions:

  • Catheter ablation is a successful initial treatment for left atrial flutter, significantly improving symptoms.
  • Despite high acute success rates, a substantial proportion of patients experience arrhythmia recurrence within midterm follow-up.
