Background:

  • Posttraumatic dystonia (PTD) treatment efficacy with deep brain stimulation (DBS) remains uncertain.
  • Existing reports on DBS for PTD have yielded conflicting results.

Observation:

  • A case series of five patients with disabling PTD undergoing DBS therapy was analyzed.
  • Clinical outcomes were assessed using the Burke-Fahn-Marsden dystonia rating scale (BFMDRS) over 12 months post-intervention.

Findings:

  • Patients received DBS targeting the globus pallidus internus (GPi), subthalamic nucleus (STN), or ventral intermediate nucleus (VIM) based on lesion location and symptom presentation.
  • All five patients demonstrated significant BFMDRS movement score improvements, ranging from 52.4% to 78.6%.

Implications:

  • Deep brain stimulation (DBS) shows promise as a safe and effective treatment for medically refractory posttraumatic dystonia (PTD).
  • Further research is warranted to confirm these findings and establish DBS as a standard PTD therapy.
