Superior mesenteric artery (SMA) syndrome, or Cast syndrome, causes small bowel obstruction due to duodenal compression. Surgical intervention provided symptom relief in a patient unresponsive to conservative treatment.

Background:
- Superior mesenteric artery (SMA) syndrome, also known as Cast syndrome, is a rare condition causing small bowel obstruction.
- It results from compression of the third part of the duodenum between the superior mesenteric artery and the aorta, leading to duodenal outflow obstruction.
Observation:
- A 46-year-old male presented with worsening chronic abdominal pain, nausea, and vomiting, exacerbated by food intake.
- Computed tomography (CT) of the abdomen and pelvis demonstrated gastric and proximal duodenal dilatation.
- The CT scan confirmed compression of the third part of the duodenum by the SMA and aorta.
Findings:
- Conservative management, including total parenteral nutrition, was unsuccessful in alleviating the patient's symptoms.
- The patient subsequently underwent a gastrojejunostomy procedure.
- Following the surgery, the patient experienced significant relief from his presenting symptoms.
Implications:
- Cast syndrome, though rare, is an important consideration in patients presenting with symptoms like abdominal pain, vomiting, early satiety, and weight loss.
- CT imaging is crucial for diagnosing SMA syndrome by visualizing duodenal compression.
- Upper gastrointestinal endoscopy plays a role in excluding other potential causes of duodenal obstruction and gastric dilatation.
