    Summary

    Sleep apnea (SA) patients show reduced common carotid blood flow (CCF) and increased resistance index (RI) during apneic episodes. These changes suggest vasomotor responses to cerebral ischemia may drive SA, with periodic breathing being secondary.

    Background:

    • Sleep apnea (SA) is associated with cardiovascular changes.
    • Noninvasive Doppler techniques allow measurement of cerebral blood flow dynamics.

    Purpose of the Study:

    • To investigate changes in common carotid blood flow (CCF) and resistance index (RI) during apneic episodes in SA patients.
    • To compare these changes with those during voluntary breath holding in SA patients and normal subjects (N).

    Main Methods:

    • Pulsed Doppler technique used to measure CCF and RI from velocity waveforms.
    • Measurements taken during spontaneous apneic episodes and voluntary breath holding.

    Main Results:

    • During apneic episodes (avg. 27s), CCF decreased by 9% and RI increased by 4% in SA patients, correlated with apneic duration.
    • CCF significantly decreased in SA patients during breath holding, unlike in normal subjects.
    • Internal carotid artery showed more pronounced changes than CCF in one SA patient.

    Conclusions:

    • Observed CCF and RI changes during SA resemble those in animal models with induced vasomotor waves.
    • Vasomotor responses to intermittent cerebral ischemia and hypercapnia are proposed as the primary mechanism in SA.
    • Periodic breathing may be a secondary consequence of autonomic dysfunction in SA.
