Summary

Optic nerve hemangioblastomas are rare tumors. A high suspicion is needed for diagnosis, especially when Von Hippel-Lindau (VHL) syndrome is absent.

  • Optic nerve hemangioblastomas are rare tumors.
  • They are typically associated with Von Hippel-Lindau (VHL) syndrome.
  • Sporadic occurrences have been reported.

Observation:

  • A 33-year-old woman presented with progressive vision loss in her right eye.
  • Fundoscopy revealed optic atrophy and a relative afferent pupillary defect.
  • Imaging demonstrated an enhancing lesion in the optic nerve with flow voids.

Findings:

  • A reddish lesion involving the right optic nerve proximal to the chiasm was identified.
  • The lesion had multiple feeding vessels.
  • Surgical excision was performed, sacrificing the optic nerve.

Implications:

  • Optic nerve hemangioblastoma is a rare entity.
  • A high level of suspicion is crucial for preoperative diagnosis, particularly in the absence of VHL syndrome.
