Summary

Higher plasma bilirubin levels are linked to reduced total and cancer mortality, particularly in men. This antioxidant effect was not observed for cardiovascular disease mortality.

Background:

  • Bilirubin, an endogenous antioxidant, is hypothesized to protect against oxidative stress-related diseases like cardiovascular diseases (CVD) and cancer.
  • Investigating the link between bilirubin levels, UGT1A1 gene variations, and mortality provides insights into disease prevention.

Purpose of the Study:

  • To examine the relationship between plasma bilirubin concentrations and UGT1A1 gene variations with total, CVD, and cancer mortality.
  • To determine if bilirubin's antioxidant properties offer protection against mortality from major diseases.

Main Methods:

  • A nested case-control study within the Polish HAPIEE cohort.
  • Analysis of plasma bilirubin and UGT1A1 promoter gene variations in 447 deceased cases and 1423 survivors.
  • Mortality follow-up via linkage with national and regional death registers.

Main Results:

  • A significant negative association was found between higher plasma bilirubin levels and total mortality (OR 0.61) and cancer mortality (OR 0.39), especially in men.
  • No significant association was observed between bilirubin levels and cardiovascular disease mortality.
  • The UGT1A1*28 allele, a marker for higher bilirubin, showed a non-significant negative association with total and cancer mortality.

Conclusions:

  • Observational and genetic data support a protective role of bilirubin against total mortality, primarily driven by reduced cancer mortality.
  • The protective association of bilirubin against cardiovascular disease mortality was not evident in this study.
