Reporter Genes

Reporter genes are a type of protein-coding gene that are often tagged to a gene of interest. Once inside a target cell, reporter genes usually produce visually identifiable characteristics like fluorescence and luminescence when expressed along with the gene of interest. Thus, reporter genes “report” the presence or absence of genes of interest in an organism, determine the gene expression pattern, or track the physical location of a DNA segment or protein in the cell.

Background:

Gingival enlargements (GEs) are multifactorial, with medications being a significant etiological factor.
Common drug culprits include ciclosporin, phenytoin, and calcium channel blockers (CCBs).
This report focuses on a specific CCB, Amlodipine, and its association with severe GEs.

Observation:

A 56-year-old male on long-term Amlodipine for hypertension developed severe gingival enlargement affecting both arches.
The enlargement led to significant dental malposition and required extensive tooth extraction.
Histological examination revealed non-specific inflammation, predominantly lymphocytes.

Findings:

Cessation of Amlodipine resulted in significant reduction of the gingival enlargement.
Despite treatment, the severity necessitated extraction of most teeth.
No recurrence of gingival enlargement was observed one year post-cessation.

Implications:

Clinicians must maintain vigilance for drug-induced gingival enlargements in patients using CCBs.
Early recognition and management of CCB-induced GEs can prevent severe dental complications.
This case underscores the potential for significant adverse effects of commonly prescribed antihypertensive medications.
