Background:

  • High-risk papillary thyroid microcarcinomas (PTMCs) often require immediate surgical intervention.
  • The anatomical location of PTMCs within the thyroid lobe may influence disease aggressiveness and treatment decisions.

Purpose of the Study:

  • To investigate the association between PTMC location (subcapsular vs. nonsubcapsular) and aggressive tumor biology.
  • To determine if PTMC location impacts the necessity of immediate surgical treatment.

Main Methods:

  • Retrospective analysis of 288 patients who underwent initial surgery for PTMC.
  • Comparison of subcapsular thyroid microcarcinomas (STMs) and nonsubcapsular thyroid microcarcinomas (NSTMs) based on ultrasound characteristics, tumor size, extrathyroidal extension, lymph node metastasis, and multifocality.
  • Assessment of preoperative ultrasound for predicting recurrent laryngeal nerve (RLN) involvement.

Main Results:

  • No significant differences in tumor size, multifocality, or lymph node metastasis were observed between STM and NSTM groups.
  • Macroscopic extrathyroidal extension was exclusively identified in the STM group.
  • Preoperative ultrasound demonstrated 100% sensitivity for predicting RLN involvement in STMs, though specificity and positive predictive value were 43.3%.

Conclusions:

  • Subcapsular location of PTMCs is associated with a higher risk of extrathyroidal extension.
  • Immediate surgery is recommended for high-risk subcapsular PTMCs due to potential aggressive behavior and RLN involvement.
