    Summary

This study developed a multivariate model to predict recent Pap smear screening. Medical care utilization, not sociodemographics or race, emerged as the strongest predictor of timely Pap smear use.

    Area of Science:

    • Public Health
    • Preventive Medicine
    • Health Services Research

    Background:

    • Limited bivariate analyses exist for women's Pap smear screening recency.
    • Understanding predictors of screening behavior is crucial for public health interventions.

    Purpose of the Study:

    • To develop a multivariate model predicting the recency of Pap smear screening.
    • To identify key factors influencing timely Pap smear utilization among women.

    Main Methods:

    • Systematic sampling of women aged 25-74 in upstate New York.
    • Data collection on Pap smear history, medical practice congruence, healthcare utilization, and sociodemographics.
    • Development of a log linear weighted least squares regression model.

    Main Results:

    • The multivariate model explained 30% of the variance in Pap smear screening recency.
    • Medical care utilization variables were the strongest predictors of recent Pap smear use.
    • Sociodemographic variables were important but secondary predictors; race was not a significant predictor.

    Conclusions:

    • Multivariate modeling provides a robust approach to understanding Pap smear screening behavior.
    • Healthcare access and utilization are critical determinants of adherence to screening guidelines.
    • Future interventions should focus on enhancing medical service use to improve Pap smear screening rates.
