    Background:

    • Anorexia nervosa (AN) shares clinical symptoms with hypothyroidism.
    • Previous studies reported low circulating thyroid hormones (T4, T3) in AN, but inconsistent thyroid-stimulating hormone (TSH) and thyrotropin-releasing hormone (TRH) responses.
    • The hypothalamic-pituitary-thyroid axis in AN requires further elucidation.

    Purpose of the Study:

    • To comprehensively assess thyroid function and the hypothalamic-pituitary axis in women with anorexia nervosa.
    • To investigate the impact of weight gain on thyroid parameters in AN patients.

    Main Methods:

    • Serum levels of free and total thyroid hormones (T4, T3), binding proteins (TBG, TBPA), TSH, and reverse T3 (rT3) were measured using a highly sensitive assay in 21 AN patients and normal controls.

    Conclusions:

    • Anorexia nervosa is associated with significant alterations in thyroid hormone levels and TSH responsiveness, indicative of hypothalamic hypothyroidism.
    • These thyroid function abnormalities in AN appear to be an adaptive response to prolonged starvation.
    • Weight restoration leads to the normalization of most thyroid parameters, suggesting the reversibility of this adaptive state.
