Riedel thyroiditis, a rare thyroid fibrosis, is difficult to diagnose with imaging. Pathology confirms Riedel thyroiditis, but elevated IgG4 levels may offer diagnostic clues.

Area of Science:

  • Endocrinology
  • Pathology
  • Immunology

Background:

  • Riedel thyroiditis is a rare, chronic inflammatory thyroid condition.
  • Characterized by dense fibrosis replacing normal thyroid tissue.
  • Literature primarily contains individual case descriptions.

Observation:

  • Presents an euthyroid patient with multinodular goiter.
  • Diagnosis of Riedel thyroiditis was confirmed via pathology.
  • Imaging and thyroid function tests were insufficient for diagnosis.

Findings:

  • Imaging modalities (ultrasound, CT, MRI, radionuclide) lack diagnostic specificity.
  • Standard thyroid function tests do not confirm Riedel thyroiditis.
  • Elevated IgG4 levels may be a helpful diagnostic indicator.

Implications:

  • Highlights the diagnostic challenges of Riedel thyroiditis.
  • Emphasizes the critical role of histopathological examination for definitive diagnosis.
  • Suggests IgG4 measurement as a potential adjunct in diagnosis.
