    Background:

    • Kidney injury can lead to significant morbidity and mortality.
    • The role of intestinal microbiota in modulating kidney injury response is not fully understood.

    Purpose of the Study:

    • To compare the effects of kidney trauma on germfree (GF) and conventional (CONV) rats.
    • To investigate the protective role of the absence of intestinal flora in acute kidney injury.

    Main Methods:

    • Induction of kidney trauma via unilateral nephrectomy and contralateral renal artery ligation in GF and CONV rats.
    • Assessment of survival rates, renal blood flow, blood urea nitrogen levels, urine concentrating ability, and thiourea clearance.

    Main Results:

    • GF rats exhibited significantly lower mortality rates (25%) compared to CONV rats (60%) post-kidney trauma.
    • Renal blood flow was preserved in GF rats, while CONV rats showed a 70% reduction.
    • Blood urea nitrogen levels were markedly elevated in CONV rats (28x normal) compared to GF rats (4x normal).
    • Kidney concentrating ability was maintained in GF rats but lost in CONV rats.

    Conclusions:

    • The absence of intestinal flora (germfree state) confers significant protection against kidney trauma.
    • Reduced microbial burden and potential auxiliary dialysis by an enlarged cecum in GF rats may contribute to improved outcomes.
    • Findings suggest the potential benefit of oral antibiotic therapy in managing human uremia.
