Background:

  • Diffuse prostate cancer (PCa) can be challenging to detect on MRI, especially with diffusely T2 hypointense peripheral zones (PZs).
  • Accurate detection of diffuse PCa is crucial for effective treatment planning.

Purpose of the Study:

  • To evaluate the accuracy of the obscured surgical capsule (OSC) sign for detecting diffuse PCa.
  • To assess OSC's predictive value in cases with diffusely T2 hypointense PZs on prostate MRI.

Main Methods:

  • Retrospective analysis of prostate MRI scans (2015-2017).
  • Two radiologists blindly assessed T2-weighted sequences for T2 hypointense PZs, OSC, and diffusion abnormalities.
  • Diffuse PCa defined by biopsy results (≥4 contiguous cores, 2/3 levels).
  • Interobserver agreement and multivariate logistic regression analysis were performed.

Main Results:

  • Seven out of 70 patients (10%) had diffuse PCa.
  • Obscured surgical capsule (OSC) showed 57% sensitivity and 89% specificity for diffuse PCa.
  • OSC was significantly associated with diffuse PCa (OR, 17.6; P = 0.0008).
  • Multivariate analysis identified OSC as the sole significant predictor of diffuse PCa.

Conclusions:

  • The obscured surgical capsule (OSC) is a significant independent predictor of diffuse prostate cancer (PCa).
  • OSC is valuable for diagnosing diffuse PCa in the context of T2 hypointense peripheral zones on MRI.
