This lesson deals with the crossed aldol reaction using weak bases. The self-condensation of an aldehyde having α hydrogen is prevented by adding it slowly to a mixture of formaldehyde and weak bases like hydroxide and alkoxide. Upon slow addition of the aldehyde, the base deprotonates the α carbon of the aldehyde to form the corresponding enolate. The enolate subsequently attacks the formaldehyde to form a single crossed product. Figure 1 depicts the aforementioned reaction.

Purpose of the Study:

  • To compare the trade-offs of risk-based versus age-based screening for women aged 45 and older.
  • To determine short-term outcomes of different mammography screening strategies in women aged 40-49.

Main Methods:

  • Retrospective cross-sectional study of 20,539 digital screening mammograms from 10,280 women aged 40-49.
  • Comparison of two hypothetical scenarios: age-based (≥45 years) and risk-based (5-year risk > average 50-year-old).
  • Risk factors included family history, race, age, prior biopsy, and breast density.

Main Results:

  • Age-based screening (≥45) detected more cancers (68%) but also more false positives (50.3%) and benign biopsies (45.6%) than risk-based screening in women 40-49.
  • Risk-based screening showed low eligibility in the 40-44 age group.
  • Differences in the 45-49 age group drove the overall hypothetical screening strategy differences.

Conclusions:

  • Risk-based screening for women 40-49 years old has low eligibility in the 40-44 age range.
  • Significant trade-offs in the 45-49 age group explain differences between screening scenarios.
  • Both strategies alter the benefits and harms of mammography for women aged 40-49.
