Low frequency intermuscular coherence (IMC) shows promise as a biomarker for predicting Globus Pallidus interna Deep Brain Stimulation (GPi-DBS) efficacy in dystonia patients. This finding may help personalize treatment for better outcomes.
Area of Science:
- Neuroscience
- Biomarkers
- Neuromodulation
Background:
- Dystonia treatment response to Globus Pallidus interna Deep Brain Stimulation (GPi-DBS) varies significantly due to disease heterogeneity.
- Predicting individual patient response to GPi-DBS remains challenging.
Purpose of the Study:
- To identify a non-invasive biomarker for predicting GPi-DBS efficacy in dystonia patients.
- To investigate the relationship between intermuscular coherence (IMC) and corticomuscular coherence (CMC) with dystonia severity and GPi-DBS outcomes.
Main Methods:
- EEG-EMG recordings were obtained from twelve dystonia patients pre- and 1-year post-GPi-DBS surgery, both ON and OFF stimulation.
- Dystonia severity was assessed using the BFMDRS and TWSTRS.
- Intermuscular coherence (IMC) and corticomuscular coherence (CMC) in low frequency (4-12 Hz) and beta bands (13-30 Hz) were calculated.
Main Results:
- GPi-DBS therapy significantly improved dystonia severity (BFMDRS: 30%, TWSTRS: 22%).
- Peak low frequency and beta band IMC significantly decreased after GPi-DBS.
- IMC levels, particularly in the low frequency band, showed a partial correlation with dystonia severity and improvement.
Conclusions:
- Peak low frequency IMC can serve as a potential biomarker for GPi-DBS efficacy in dystonia.
- Further validation in larger cohorts is required for clinical application of IMC as a predictive biomarker.
