Background:

  • The tectorial membrane (TM) is crucial for converting acoustic energy into mechanical signals.
  • Pathological changes in the TM can lead to hearing loss (HL).

Purpose of the Study:

  • To investigate morphological changes in the human tectorial membrane (TM).
  • To correlate TM abnormalities with hair cell and interdental cell degeneration and endolymphatic hydrops (EH) in sensorineural hearing loss (HL).

Main Methods:

  • Histopathological evaluation of 2,290 human temporal bone (TB) specimens.
  • Analysis of morphological TM abnormalities, hair cell status, interdental cells, and presence of EH.
  • Exclusion of TBs with cochlear trauma or severe postmortem artifacts.

Main Results:

  • 164 cases met inclusion criteria, with idiopathic sudden deafness, genetic causes, and ototoxicity being most common.
  • Tectorial membrane (TM) abnormalities included deformed, shrunken, detached, encapsulated, or missing patterns.
  • Endolymphatic hydrops (EH) was present in 33.3% of cases with TM abnormalities.

Conclusions:

  • A high prevalence of EH in TM abnormality cases suggests a shared pathophysiology.
  • Anatomical TM abnormalities may contribute to the development of hearing loss (HL).
